Health Information Exchange

Health Information Exchange (HIE)

St. Joseph’s Health, along with other healthcare providers in New Jersey, participates in multiple Health Information Exchange programs (HIE). A HIE allows patient information to be shared electronically through a secured network that is accessible to the providers treating you. We may disclose your Patient Health Information (PHI) to the HIE unless you opt out of participating. To opt out, please inform a St. Joseph’s Health staff member during registration or check-in at any of the St. Joseph’s Health facilities. Should you decide to opt out after your visit, please email us (sjhhie@sjhmc.org). Please provide your current contact information including name, date of birth and preferred email address. St. Joseph’s Health Primary Care Office providers can be contacted directly to make any updates or changes.

How information will be used?

Protected Health Information (PHI) about you that is obtained through the St. Joseph’s Health HIE will be used for treatment only. Policies that follow federal and state law limit how your information can be used or disclosed through the St. Joseph’s Health HIE. The privacy notices distributed by each individual provider explains how those providers use the PHI that those providers have and collect about you independent of the HIE.

Who can access my records through the St. Joseph's Health HIE?

Only health care providers who are participants in the St. Joseph's Health Commonwell Alliance and NJHIN HIE Network, and their associated staff, who are treating you, are specifically given rights to access your PHI.

How secure is the St. Joseph's Health HIE?

There are data encryption, audit logs and security features in place to keep your record secure. Even with security protocols in place, in an event of a breach, you will be notified, and the SJH Commonwell Alliance and NJHIN HIE Network and other participating providers will take necessary steps to correct the problem.

Can I access my own medical records?

My St.Joseph’s Record is a free and secure electronic patient portal for St. Joseph’s Health patients. Through My St. Joseph’s Record patients can access their medical records, view test results, schedule appointments and privately message their care team.
