Supportive Housing Program

The Supportive Housing Program is a case management service for 120 persistently and chronically mentally ill adults with a history of homelessness, residing in transitional living environments. These individuals are supported in their initial transition to independent living. Caseworkers assist all consumers with identifying and obtaining permanent living arrangements and facilitating tenant-landlord relationships. All consumers are provided with the support and services necessary to sustain their independent and permanent living environments. Case management services are delivered to the extent required by the individual to maintain independent living for the long term.
