Once a decision is made for hospice services, the staff at St. Vincent's Nursing Home will arrange a meeting with the resident, their family, and the hospice agency to discuss hospice services to be provided at St. Vincent's.
As a team, St. Vincent's Nursing Home's staff and the hospice staff develop an individualized plan of care, focusing on the comfort of the resident. We encourage family members to attend frequently scheduled meetings. Realizing that they are also going through a difficult time, the staff at St. Vincent's Nursing Home also provides comfort and assistance to family members.
