Based at St. Joseph’s Regional Medical Center in Paterson and funded by the NJ State Division of Mental Health Services, ACCESS provides community-based mental health services to deaf and hard of hearing people throughout New Jersey. The offices are located at 60 Hamilton Street in Paterson, New Jersey.
ACCESS Integrated Case Management Services (ICMS) are provided to deaf and hard of hearing individuals who are deemed ready for discharge in the state psychiatric hospital, and is provided to certain individuals living in the community, who have been determined to be at higher risk for psychiatric hospitalization.
The Case Manager attends weekly clinical rounds meetings at the hospital to keep abreast of new developments with various patients. At that time, linkages to community services and agencies begin. Upon discharge from the hospital, individuals are followed with ICMS services for up to 18 months in the community. Again, services are recovery-based, focusing on building skills for successful community living.
